Filing a Complaint

If you believe a student group has violated University policy or otherwise acted unethically, the Formal Complaint Statement is the official vehicle for notifying the Student Activities Office.
A Formal Complaint Statement must be submitted in writing to the Student Activities Office. Complaint Forms are available at the Student Activities Office or can be downloaded from this page. The complaint must identify:
- The complainant including; email, phone, mailing address, contact information.
- The respondent group
- Individuals involved
- The incident or action at issue
- The policy, regulation, or established practice alleged to be violated
- A brief statement of the redress sought.
